Terms & Conditions


A member may request to cancel their plan within the first thirty (30) days after the effective date of membership (or delivery of the membership kits in IN, CT, MO, MT, WA), and will receive reimbursement of all periodic charges upon return of the discount cards to CHCS Services. After the initial 30 day period, a member may still cancel the Agreement; however, if a member cancels and has utilized the telephonic or in-home assessment tools the member is responsible for the full price of these services (except in WA).

Your notice of cancellation must be done in writing and mailed to:

Refunds will be issued within ten (10) days of receipt of your notice of cancellation, for cancellations occurring within three (3) business days of the date you sign your application and for all other cancellations, refunds will be issued within thirty (30) days of receipt of your notice of cancellation. CHCS will cease collecting membership fees in a reasonable amount of time, but no later than 30 days after receiving a valid cancellation notice.

Once CHCS receives your notice of cancellation, you are not obligated to make any further payments, nor are you entitled to any further benefits of this Agreement for any period following the last month in which payment has been made.
